Nature of the Project: What is to be accomplished?

Project Title

Address/Location of Project

Contact Name (the person in charge of the site)

Contact Information (phone number and email)

Working with any other groups (e.g., Boy or Girl Scouts, School Children, Seniors)

Approximate number of hours to be worked by Master Gardener in Administration (planning), Outreach (person-to-person teaching) and gardening
